How would you like to have lunch on us??? We'll give you a $25.00 gift certificate for lunch at Mallie Kyla's in Brooksville.
All we ask is that you suggest us for eye exams for your friends and family members. When you have referred three new patients to our office and they come in and complete an eye examination, we'll send you the gift certificate as a thank you gift.
All you need to do is download the PDF on the Forms page and print the referral form, fill out your name and the name of your referral, then give it to your new referral to bring in to us at their examination. When the third new patient is seen, you get the gift certificate. It's as easy as that!
